50,000 Help for Family died from Covid-19 Application Form Scheme Details

The State Government of Uttar Pradesh has started a scheme to provide financial assistance of ₹ 50,000/- to the next of kin of those who died from Covid-19.


How to apply for getting financial assistance of ₹ 50,000/-

The application form can be filled online by clicking on the option "Ex-gratia assistance of ₹50,000/- to the kin of those who died due to infection of Covid-19" on the website of the Relief Commissioner's office https://rahat.up.nic.in .

You will have to fill the information on the portal for online application, for this, OTP will be received.

After filling the OTP, other columns of the application form can be filled. Any one of the following records has to be uploaded along with the online application form.


RTPCR rapid antigen test

  • Molecular test
  • Copy of medical treatment/medical examination in which COVID-19 is diagnosed
  • In addition to the above, the following records will also have to be uploaded:
  • death certificate of the deceased
  • Consent letter of heirs
  • The family who want to get the assistance amount.
  • Photocopy or cross check of his bank passbook

In addition to the above, the application form in hard copy can be given in the "Covid 19 Non-helpful Application Form Receipt Cell" located in the District Collector's Office (Collectorate) of the concerned district, to which the above records will also have to be attached.

Along with this, if any applicant / heir has any problem in submitting the application form or has any objection to the decision taken on the application form, then the applicant should submit the representation before the committee constituted under the chairmanship of Additional District Magistrate (V/R) in his district. or can lodge a complaint on the website of the Relief Commissioner's office https://rahat.up.nic.in or submit a complaint to the District Magistrate concerned.
